Following Documents Duly Attested are to be Submitted Along with the Application
i) Permanent Residential Certificate and Address proof documents
ii) Mark sheet & Pass Certificate of class X Examination from respective Boards & HS (10+2) (Science) Examination
iii) Admit Card or others documents in support of age proof
iv) A copy of attested recent passport size photograph
Application in sealed enveloped with above mentioned information and documents must reach the office of the Director of Medical Education, Assam, Sixmile, Khanapara, Guwahati-22 (Drop Box) 2nd floor Room 5 on or before 25th October 2021, Application by post shall not be considered
